Background:
InsertionalAchillestendinopathyandretrocalcanealbursitisisdifficulttotreat,andseveraloperativetechniqueshavebeenusedafterfailureofconservativemanagement.DorsalwedgecalcanealosteotomyhasbeendescribedforthetreatmentofinsertionalAchillespathology.ItwashypothesizedthatdorsalwedgecalcanealosteotomywouldbeaneffectiveandsafemethodforthetreatmentofathleteswithinsertionalAchillespathologyunrelievedbynonoperativemeasures.
Methods:
Fifty-twoathletes(64feet)whohadpainfulAchillestendonsyndromeunrelievedby6monthsofnonoperativemeasuresweretreatedsurgically.Dorsallybasedwedgecalcanealosteotomywasperformedthroughalateralapproach,and2stapleswereusedforfixation.Patientswerescoredpre-andpostoperativelywiththeAmericanOrthopaedicFootAnkleSociety(AOFAS)ankle-hindfootandVictorianInstituteofSportsofAustralia-Achilles(VISA-A)scores.
Results:
Ataminimumfollow-upof3years,thepatients’AOFASandVISA-Ascoresimprovedfrom59.5±15.0and65.9±11.1preoperativelyto95.7±6.2and90.2±8.4postoperatively,respectively.Clinicalresultswereconsideredexcellentin38patients,goodin12patients,andfairin2patients.Returntoprevioussportsactivitytimewas21(SD,8.0)weeks.Onepatientnecessitatedarevisionoperation.
Conclusion:
OperativetreatmentofinsertionalAchillespathologyinathleteswithdorsalclosingwedgecalcanealosteotomywasasafeandeffectivemethodthatallowedforaquickerreturntopreviouslevelofsportsactivities白癜风医生白癜风康复来袭
